In 2045, Climate change might result in increasing pressure on the health sector. Both because of higher number of people due to refugee movement, but also due to heatwaves, higher numbers of natural disasters, etc. 

Globally, this might mean that countries with an unstable health sector find themselves under even more pressure because 1) they have to deal with climate refugees but are not equipped for it, 2) they are not equipped for crisis management in the case of natural disasters and high numbers of patients all at the same time. Resource scarcity plays into this as well which might easily lead to conflicts. 

For the UK, this means either higher numbers of refugees coming to the UK because they can't find stable conditions elsewhere. This might also put the NHS under more pressure as under-funded as it already is. Or working worldwide to provide aid to countries affected by climate change or affected by the increased mobility due to climate change. Opening up to work increasingly with non-state actors. 

And not to forget the impacts of climate change in Britain itself, heatwaves and storms, which might put more people into hospital.
